This is the Diabetes Clinical Pathways Case Study Test. Please complete all 10 questions. A passing score is 70%. If you score lower than 70%, please take the test again until you pass. To take the test again, just click on the "X" in the upper right corner of the screen and click on the "Diabetes Clinical Pathways Case Study Test" again on the Education Website.

This test is to be completed by VNA admission specialists and LPN's after attending the Diabetes Pathways Workshop. RN's should complete the written pathways module checklist as their case study and return to Karen DiPietrok, RD, CDE. OT's and PT's are exempt.

You do not need to print the test or notify Staff Development when you have taken the test. Staff Development will be checking test results weekly, printing your certificate and giving the certificates to Human Resources to file.
